We, the undersigned, hereby make application to volunteer for Community Renewal International ("CRI") programs. In consideration of permission for the youth to participate in this, the undersigned hereby release, hold harmless and forever discharge CRI, its officers, directors, agents, employees, representatives, volunteers, underwriters and insurers, from any and all claims, charges, demands, suits, liens, rights, and causes of action resulting from my participation in any CRI projects or programs regardless of the cost or whether the damage, loss, or injury is to person or property, or both.

The undersigned parent/guardian warrant that the youth has never been convicted of a crime and hereby warrant that the youth has not demonstrated any disciplinary problems or other behavior which could reflect upon or cast doubt upon their ability to adhere to and abide by the "Rules and Expectations."

We do hereby further state that the youth is physically fit and suffering from no condition, impairment, disease, infirmity, or other illness which would prevent my participation in any of these activities or programs. We give CRI permission to use photographs, and/or video, and/or audio of this youth obtained while participating in CRI programs.

This release shall be binding on my heirs, executors, administrators, personal representatives, successors, and assigns.
